🌱 Diabetes Prevention Program

A proven, CDC-recognized lifestyle change program to help prevent or delay type 2 diabetes.

Did You Know? Over 96 million American adults have prediabetes — and more than 80% don’t know it. The good news: type 2 diabetes can often be prevented with small lifestyle changes.

About the Program

The Northeast Nebraska Public Health Department offers the CDC-recognized National Diabetes Prevention Program (DPP) — a year-long lifestyle change program designed to help people with prediabetes prevent or delay the onset of type 2 diabetes.

Led by trained lifestyle coaches, participants meet in a small group setting to work on healthier eating habits, increasing physical activity, and managing stress. The program has helped participants lose an average of 5–7% of their body weight, which can cut diabetes risk by more than half.

Who Is Eligible?

You may be eligible if you:

  • Have been told by a doctor that you have prediabetes
  • Had gestational diabetes (diabetes during pregnancy)
  • Are overweight and age 45 or older
  • Are overweight, under 45, and have at least one additional risk factor (family history, high blood pressure, low HDL cholesterol)
  • Score 5 or higher on the CDC Prediabetes Risk Test

What to Expect

Year 1 Schedule

16 weekly core sessions, then 6 monthly sessions for a full year of support.

Healthy Eating

Learn practical strategies for healthier food choices without giving up foods you love.

Physical Activity

Work toward at least 150 minutes of moderate activity per week at your own pace.

Stress Management

Coping strategies and peer support to help you make lasting, sustainable changes.

Spring 2026 Session Starts March 10th. Tuesdays, 5:30–6:30 PM at the Main Office Community Room. Space is limited — register by March 3rd to secure your spot.

The DPP is covered by Medicare, Nebraska Medicaid, and many private insurance plans at little or no cost to you. For uninsured participants, a sliding scale fee may be available. Call us to discuss your specific situation.

No — the program is specifically designed for people with prediabetes, not those who already have type 2 diabetes. The goal is prevention and delay before diabetes develops.

Yes! We offer both in-person and virtual options via Zoom to accommodate different schedules and locations. Please indicate your preference when registering.
