About the Program
The Northeast Nebraska Public Health Department offers the CDC-recognized National Diabetes Prevention Program (DPP) — a year-long lifestyle change program designed to help people with prediabetes prevent or delay the onset of type 2 diabetes.
Led by trained lifestyle coaches, participants meet in a small group setting to work on healthier eating habits, increasing physical activity, and managing stress. The program has helped participants lose an average of 5–7% of their body weight, which can cut diabetes risk by more than half.
Who Is Eligible?
You may be eligible if you:
- Have been told by a doctor that you have prediabetes
- Had gestational diabetes (diabetes during pregnancy)
- Are overweight and age 45 or older
- Are overweight, under 45, and have at least one additional risk factor (family history, high blood pressure, low HDL cholesterol)
- Score 5 or higher on the CDC Prediabetes Risk Test

Year 1 Schedule
16 weekly core sessions, then 6 monthly sessions for a full year of support.

Coping strategies and peer support to help you make lasting, sustainable changes.
