Northeast Nebraska Public Health Department serves as a resource for licensing and registration information, helping residents and businesses connect with the right state and federal agencies for their needs.

Nebraska Department of Health & Human Services (DHHS)

Nebraska DHHS handles most professional health licensing, facility licensing, and food establishment registration in our state.

Professional Licenses

Nursing, medical, dental, mental health, pharmacy, and allied health professional licenses are issued by Nebraska DHHS.

Healthcare Facilities

Hospitals, long-term care facilities, assisted living, home health, and ambulatory surgical centers are licensed by DHHS.

Food Establishments

Restaurants, retail food stores, mobile food units, and food processing facilities register with DHHS Food Safety Division.

Childcare Licensing

In-home and center-based childcare facilities are licensed by DHHS Childcare Licensing.

Vital Records & Certificates

Birth certificates, death certificates, marriage licenses, and divorce records are managed through Nebraska Vital Records. See the Vital Records section below for details.

Body Art & Cosmetology

Tattoo artists, body piercers, cosmetologists, barbers, and estheticians are regulated and licensed through Nebraska DHHS and the Board of Cosmetology, Electrology, Esthetics, Nail Technology and Body Art.

Environmental Permits

Septic systems, well drilling, and other environmental permits are typically handled at the county level. Contact your county zoning office for specific requirements.

While NNPHD does not directly issue most licenses, we can help you:

Vital Records

Vital records include certified copies of birth certificates, death certificates, marriage licenses, and divorce records. These official documents are needed for many life events — enrolling in school, applying for a passport, settling an estate, or proving identity.

Birth Certificates

Certified copies of Nebraska birth certificates can be ordered from Nebraska Vital Records. Expect to provide:

Death Certificates

Certified death certificates are typically needed for estate settlement, insurance claims, and benefits. Order through Nebraska Vital Records — funeral homes often help families obtain initial copies at the time of passing.

Marriage Licenses

Marriage licenses are issued by the county clerk in the Nebraska county where you plan to marry. Requirements include:

Divorce Records

Divorce decrees are filed with the District Court in the county where the divorce was finalized. Certified copies can also be obtained from Nebraska Vital Records for divorces finalized after 1909.

Records for vaccines administered by NNPHD can be requested by calling (402) 375-2200 or visiting our office with photo ID. For vaccines from other providers, contact the Nebraska Immunization Registry (NESIIS) through your provider or Nebraska DHHS.
Yes. NNPHD is a local governmental public health agency governed by an elected Board of Health and authorized under Nebraska state statute. We receive funding from local property taxes, state allocations, federal grants, and program fees.
Submit requests in writing to 801 Milo Drive, Wayne, NE 68787 or email info@nnphd.ne.gov. We respond within 4 business days per Nebraska public records law.
